Department Use Only
The following forms are for department use only. International scholars must coordinate with their department administrator and/or OIS prior to submitting these forms. Requests submitted without proper authorization will be rejected. For more information, please contact your immigration specialist.

To access the O-1 Request Cover Sheet or the US Lawful Permanent Resident Request Cover Sheet you must first schedule an appointment with the immigration specialist assigned to your school/department/center.
