On-line Department Request Form (DRF) Supplement (for Professor,
Research Scholar, and Short-term Scholar J-1 categories) (NOTE:
This form has changed. Please read below carefully.)

Please note that the OIS On-line Department Request Form (DRF) is
the only means by which an academic department can process requests
for J-1 Scholars with OIS. An international cannot submit a request
on his/her own. You must work through your department administrator
who will complete the DRF. The DRF enables department administrators
to input data and manage requests relative to J scholars via a secure
Internet connection with the OIS database system called i1440. While
departments must still print the final data screens in order to obtain
the required signatures, the process is much more streamlined and will
result in shorter processing times in OIS. This link above is to a supplemental
form that provides detailed information about the DRF process, as well
as a form to help departments facilitate collection of data from the
international for use in the DRF. This supplement may NOT be used in
place of the DRF. Please read the instructions carefully. - J-1 Scholar Transfer-In Form
